Informed Approval
Your approval is imperative to carry out few treatments and procedures. In a case where you are found incapable of making logical final decisions by yourself
due to mental incapacity, approval would be required from your guardian / caregiver who is legally authorised to make decisions on your behalf.
In situations where both options are not available, a Doctor would take the responsibility of administering the best treatment in your interest.

While under our care, you may request for spiritual support and access. We would gladly assist to provide a religious counsellor to visit and/or pray for you while you are in
the hospital. However, do note that permission can be granted only when it does not affect the peace and well-being of others.

Provision of Information
Please note, complete and accurate information regarding your health status from the period it started or was noticed by a caregiver, past & present health issues,
including medications or other related informations must be reported to ensure efficient treatment. This information can be gotten from either you or a family member / caretaker
